Graduation Registration Form

DECEMBER GRADUATION - BOOK NOW

To attend your Graduation Ceremony in December 2011, please complete the online booking form. When you submit your booking, a registration email and a registration ID number will be sent to your nominated email address.

To complete your booking you must print your registration email and take it together with your payment, to Student Services, Level 1, C Block or contact them on 13 72 48 with credit card details. Payments will not be processed without your registration ID number.

Online registration closes 5pm Friday 18 November 2011.
Payments need to be made to Student Services by Friday 2 December 2011.

All prices are shown in Australian dollars and include GST.

  • Graduation program
  • Academic gown hire

  • Attendance at graduation ceremony

Number of guest tickets: at $15 per person
(Please note: you do not need to purchase a ticket for yourself as it is included in the Graduate Ticket price.)
3. Graduation DVD:

Purchase a copy of your graduation ceremony: for only $30 per copy

Photography

Sheila Sissons Photography are available to take individual, group and family portraits prior to and following the ceremony. A photograph of each student with the Institute Director will be taken at certificate or diploma presentation.

After the graduation your photos will be available for purchase at www.sheilasissonsphotography.com.au/ . Visit their website for further information on the packages available.

Terms and Conditions

I understand that:

  1. Participation in the graduation ceremony is dependent upon the satisfactory completion of all course requirements. Should I prove to be ineligible; any money I have paid for gown hire and tickets will be refunded.
  2. If you decide not to attend and would like to claim a refund, you must contact marketing@southbank.edu.au with your request before 5pm Friday 25 November 2011.
  3. My guests and I may be videotaped and have no rights relating to the material, production or presentation that includes the material and that Southbank Institute:

    1. May use the recording to promote the Institute activities.
    2. May reproduce the recording in any form, in whole or a part, and distribute the works by any medium including the internet, CD Rom or other media.
    3. Will not pay me or my guests for giving the consent or for the use of my image or recording.

  4. My name may not appear in the graduation program if I register to graduate after 5pm Wednesday 26 November 2011
